Novel use of combined Dual Knife and endoloop for resection of a large pedunculated colon polyp.

چکیده
colonoscopy, in which a pedunculated polyp, 4 cm × 2.8 cm in size with a 2.5-cm wide flat stalk, was identified in the sigmoid colon (●" Fig. 1). For the safe and complete resection of the polyp, prophylactic endoclips (Hx-600–135; Olympus, Hachioji-shi, Tokyo, Japan) were applied to both sides of the stalk but appeared to be ineffective (●" Fig. 2). We then applied a detachable endoloop (MAJ-254; Olympus, Hachioji-shi, Tokyo, Japan) over the stalk (●" Fig. 3). Because it was difficult to capture the stalk by going around the large head of the polyp using a conventional diathermic snare, we used the Dual Knife (KD-650 Q/U; Olympus, Hachiojishi, Tokyo, Japan) with an electrosurgical generator (Endocut 50W; Coagulation 30W; ICC 200; ERBE Co., Tübingen, Germany) to cut the stalk under direct vision (●" Fig. 4;●" Video 1). After resection, the polyp was gently dragged into the rectum for spontaneous passage. Histological examination confirmed the polyp to be a tubulovillous adenoma without malignant change. In this presentation of a pedunculated polyp with very large head, conventional diathermic snare polypectomy with enbloc resection was impossible. Piecemeal resection of very large pedunculated polyps has been reported before [1,2]; however, we did not consider this option due to the increased risk of bleeding, and the risk of an inadequate tissue sample. The Dual Knife, with either a 2-mm (KD650L) or 1.5-mm (KD-650Q/U) cutting knife, can be used for the endoscopic submucosal dissection. The knob-shaped tip of the needle knife makes the needle less likely to slip during resection of polyps. Cutting the stalk of a polyp under direct vision is a safe method irrespective of the size of the polyp head. In conclusion, we consider this technique safe and effective for the management of such polyps in cases where conventional polypectomy techniques have failed. Endoscopy_UCTN_Code_TTT_1AQ_2AD

Needle Knife-assisted Endoscopic Polypectomy for a Large Inflammatory Fibroid Colon Polyp by Making Its Stalk into an Omega Shape Using an Endoloop
Colonic inflammatory fibroid polyp (IFP) is an uncommon benign polypoid lesion, which is composed of fibroblasts, numerous small vessels and edematous connective tissue with marked eosinophilic inflammatory cell infiltration. This condition is frequently detected in the stomach and small intestine, but uncommon in the colon. Although IFP is a benign lesion, surgical resections are performed in ...
متن کاملA rare complication of endoloop-assisted polypectomy: adhesion of snare and endoloop.
Endoloop-assisted polypectomy may minimize the risk of bleeding after endoscopic resection of large pedunculated colorectal adenomas [1–3]. To the best of our knowledge, this is the first report of a rare complication of this technique caused by adhesion of the snare and endoloop. A 67-year-old man was referred to our endoscopy unit for polypectomy of colorectal adenomas detected during a colon...

An 83-year-old man presented with a feeling of abdominal fullness and underwent colonoscopy. Two pedunculated polyps were identified in the sigmoid colon: the one on the oral side was about 20mm, while that on the anal side was a giant lesion nearly as large as the diameter of the bowel lumen (●" Fig.1a). It was judged to be difficult to perform polypectomy by snaring the latter polyp. Both pol...

A 47 year old female with papillary thyroid carcinoma was referred to our department for post-treatment whole body iodine scanning. The acquired images revealed a focal zone of intense radiotracer uptake in the right lower quadrant of the abdomen. SPECT/CT fused slices showed a focal zone of the radioiodine (131I) uptake in the right colon wall. Colonoscopy revealed a pedunculated po...

A 72-year-old man presented to our gastroenterology outpatient clinic with chronic abdominal pain, bloating, and past episodes of transient small-bowel obstruction. A computed tomography (CT) enterogram revealed a large, pedunculated subepithelial polyp in the distal ileum (●" Fig.1). A decision was made to proceed to endoscopic treatment of the polyp because of its dimensions and the obstructi...
